I certify that the account information I have provided is accurate and that I am an authorized owner/user of that account. This authorization remains in full force and effect until the Employer has received written notice from me of its termination and has had a reasonable opportunity to act on it. I understand ACH transactions must comply with U.S. law and the Nacha Operating Rules.
